  • What is Sildenafil Citrate?

The T1ρC of NOR and CIP complexes presented a bi-exponential behaviour indicating the non-homogeneous nature of the complexes, which presented at least two phases with different orders or mobility, as can be seen in Fig.

Medication Class Interaction Type Effect Clinical Significance
Nitrates Additive hypotensive effect Severe hypotension Contraindicated
Alpha-blockers Enhanced hypotension Blood pressure drop Use with caution
CYP3A4 inhibitors Increased sildenafil levels Prolonged or intensified side effects Dose adjustment may be needed
Protease inhibitors Elevated sildenafil levels Increased adverse effects Monitoring required

also performed 27Al MAS NMR experiments on that complexes; the main characteristic of the 27Al spectra is that it is sensitive to the coordination of the aluminium to oxygen. The peak at about 0 ppm, as it is shown in Fig. 4.2, was indicative of an octahedral coordination.

  • Sildenafil citrate was originally developed for hypertension.
  • It was later approved for erectile dysfunction treatment.
  • Brand names include Viagra and Revatio.

investigated the packing interactions in anhydrous (Form I) and hydrated (Form II) forms of CIP with SSNMR and computer simulations [88].

What other information should I know?

[86] showed the application, not only of 13C CP/MAS but also of 1H and 15N SSNMR and liquid state NMR to the analysis of sildenafil base and sildenafil citrate (Viagra). The differences between the 13C chemical shifts of the spectra in solution and in solid state were an indication of the rigidity and the conformational changes suffered by the compound. Through the 15N CP/MAS spectra, the existence of hydrogen bonds could be determined. The hydroxyl groups of the citrate anion were found to act as hydrogen bond donors. The zwitterionic character of compounds can also be established with SSNMR as shown in the work of Olivera et al.

What are the most common side effects of sildenafil?

[87], where the zwitterionic character of procainamide derivatives was shown by observing chemical shifts in 13C spectra. From the differences between the solid state and the solution state 13C spectra, the zwitterionic character of CIP in the solid state could be determined. The stoichiometry of the complexes was confirmed due to the higher multiplicity of the 13C signals in the spectra. The complete reaction of the pure drug to form the complexes was corroborated with the 13C CP/MAS spectra. As mentioned in Section 2.4, relaxation times are of great importance in SSNMR. Both crystal structures were determined with single-crystal XRD, which allowed the correlation of the SSNMR results with the known packing interactions.

The complete assignment of the 1H and 13C NMR spectra were done with a variety of SSNMR experiments among them 2D 1H–13C PRESTO–HETCOR, 1D 1H wDUMBO, 2D 1H–1H DQ CRAMPS.

5.2 Prolonged Erection and Priapism

1.2.5 Discovery of Drugs through Clinical Observations Sometimes a drug candidate during clinical trials will exhibit more than one pharmacological activity, that is, it may produce a side effect. This compound, then, can be used as a lead (or, with luck, as a drug) for the secondary activity. In 1947, an antihistamine, dimenhydrinate (1.22; Dramamine) was tested at the allergy clinic at Johns Hopkins University and was found also to be effective in relieving a patient who suffered from car sickness; a further study proved its effectiveness in the treatment of seasickness[21] and airsickness. [22] It then became the most widely used drug for the treatment of all forms of motion sickness. There are other popular examples of drugs derived from clinical observations.

Drug Interactions

Bupropion hydrochloride (1.23), an antidepressant drug (Wellbutrin), was found to help patients stop smoking and became the first drug marketed as a smoking cessation aid (Zyban). The impotence drug sildenafil citrate (1.24; Viagra) was designed for the treatment of angina and hypertension by blocking the enzyme phosphodiesterase-5, which hydrolyzes cyclic guanosine monophosphate (cGMP), a vasodilator that allows increased blood flow. [23] In 1991, sildenafil went into Phase I clinical trials for angina. In Phase II clinical trials, it was not as effective against angina as Pfizer had hoped, so it went back to Phase I clinical trials to see how high of a dose could be tolerated. It was during that clinical trial that the volunteers reported increased erectile function.

Does Viagra prevent ejaculation?

Given the weak activity against angina, it was an easy decision to try to determine its effectiveness as the first treatment for erectile dysfunction. Sildenafil works by the mechanism for which it was designed as an antianginal drug, except it inhibits the phosphodiesterase in the penis (phosphodiesterase-5) as well as the one in the heart (Figure 1.2). Sexual stimulation causes release of nitric oxide in the penis. Nitric oxide is a second messenger molecule that turns on (pun intended) the enzyme guanylate cyclase, which converts guanosine triphosphate to cGMP. The vasodilator cGMP relaxes the smooth muscle in the corpus cavernosum, allowing blood to flow into the penis, thereby producing an erection. With the SSNMR results correlated with the crystal structure previously known, they could analyze other similar compounds for which the single-crystals XRD could not be obtained.

  • Typical sildenafil dose ranges from 25 to 100 mg.
  • It should be taken 30-60 minutes before activity.
  • Do not exceed one dose within 24 hours.

Different approaches can be made using SSNMR, the work presented by Apperley et al. [89] showed the application of variable temperature SSNMR to the characterization of two compounds, indomethacin and nifedipine.

  • Common side effects include headache and flushing.
  • Some may experience nasal congestion or dizziness.
  • Serious side effects require immediate medical attention.

Changes in the chemical shift of the indomethacin SSNMR spectrum compared with the one in solution proved the formation of hydrogen bonds in the solid state.

8.5 Geriatric Use

Aztreonam was later repositioned as Cayston® for inhalation sildenafil citrate tablet by reformulating the solution to contain lysine rather than arginine [24]. Duloxetine is a serotonin and norepinephrine reuptake inhibitor which is marketed for major depressive disorder (Cymbalta®) in the United States and for stress urinary incontinence (Yentreve®) in Europe [1]. Trial studies revealed that duloxetine reduced painful physical symptoms in depressed patients [26], which led to its FDA approval for the management of diabetic peripheral neuropathic pain (DPNP) in 2004 [27] and fibromyalgia in 2008 [28]. Due to its effectiveness at relieving pain in DPNP and fibromyalgia patients, its effect on chronic lower back pain (CLBP) was evaluated in patients with CLBP. Double-blind studies on the analgesic effects of duloxetine showed significant improvement in patients with CLBP with a maintained analgesic effect for over 40 weeks [29,30].

Warnings for people with certain health conditions

In November 2010, the FDA approved the use of duloxetine to treat chronic musculoskeletal pain, including discomfort from osteoarthritis and CLBP [31]. Duloxetine has earned over $9 billion in revenue since its release in 2004 for all of its indications [32]. The solid-state characterization of novel pharmaceutical compounds remains an important issue. SSNMR is one of the techniques of choice to gain information when other techniques are not available. Recent reviews have shown the applications of high-resolution MAS SSNMR applied to pharmaceutical compounds [28,38].

What to do if you think you’ve taken too much Viagra

In this chapter, we mention some of the more relevant works on the field, which employ not only the routinely SSNMR techniques but also advanced pulse sequences and new applications of novel nuclei. In the past 10 years, an important number of publications [6,36] have shown the use of SSNMR together with other spectroscopic techniques, to obtain the complete characterization of recently developed pharmaceutical compounds. [36] reported the application of 13C CP/MAS to tizoxanide. The 13C CP/MAS spectra and the NQS spectra of the compounds allowed Bruno and co-workers to determine changes in the hydrogen bonds of the drug under study. They could identify the replacement of intramolecular H-bonding by intermolecular H-bonding. Nifedipine glass was analyzed with variable temperature SSNMR to investigate polymorphism and recrystallization through the 13C CP/MAS spectra.

What is Sildenafil Citrate?

However, phosphodiesterase-5 (PDE-5) hydrolyzes the cGMP, which causes vasoconstriction and the outflow of blood from the penis. During the past two decades, the drug repositioning mindset has led to the discovery of several important and profitable drugs. Such an approach often allows pharmaceutical companies to extend a drug's patent life and to reduce the cost of drug development. Sildenafil (1; Figure 1), also known as Viagra®, is one of the most notable drugs that has been repositioned. It is a phosphodiesterase (PDE) type 5 inhibitor that was originally developed to treat angina, but it was repositioned during clinical testing to treat male erectile dysfunction in order to fully take advantage of its therapeutically relevant side effect profile [15].

Can Viagra be crushed, split, or chewed?

As a result of this serendipitous repositioning, sildenafil citrate has grossed over $15 billion in revenue since its release in 1998 [16]. Another notable example is thalidomide (2), a sedative that received much notoriety in the early 1960s due to its severe teratogenic effects [17]. Despite its infamy, thalidomide was repositioned to treat erythema nodosum leprosum and was approved for the treatment of this form of leprosy by the Food and Drug Administration (FDA) in 1998 (Thalomid®) with the caveat of its teratogenic effects [18]. Moreover, thalidomide was repositioned again to treat multiple myeloma and FDA approved in 2006 in combination with dexamethasone for newly diagnosed multiple myeloma patients [19]. Some recent examples of drug repositioning include aztreonam (3) and duloxetine (4).

Is there a “female Viagra”?

Aztreonam is a monobactam antibiotic that was approved in 1986 for the treatment of Gram-negative bacterial infections via intravenous or intramuscular injection (Azactam®) [20]. This drug was marketed to treat bacterial infections and is safe for patients with penicillin allergies [21]. Previously, aztreonam was only approved for intravenous or intramuscular injection but not for inhalation because the solution for aztreonam contains an arginine buffer. Arginine is a substrate for nitric oxide production in many organs, and the generation of nitric oxide can lead to tissue injury [22]. Previous inhalation studies in patients with cystic fibrosis demonstrated that long-term use of inhaled arginine was associated with airway inflammation and deterioration of symptoms [23]. As temperature was increased, a change in the linewidth of the signals in the spectra could be seen. This fact is due to the increase mobility if the sample and the ability of recrystallization inherent in the compound. The same was done for indomethacin, but the results gave no information.
